  Thank you, parents, for your interest in sending your children to Dolores Mission School. The community, parents, teachers and staff all work hand in hand to “create men and women for and with others.” A Catholic education is an investment for a lifetime, not only for your child(ren), but also for your family.
  
  This is a registration packet for the 2024-2025 academic year.
  
  Please complete all documents and return to the school office.
  
  These documents are:
  
  Registration Intent Form (Or fill out this online section)
  
  Financial Aid Application – this must be completed to qualify for a Dolores Mission Scholarship. (English or Spanish)
  
  If a notarized letter is needed please use the form included
  
  Information for the 2022 year is needed (taxes, CalWorks, CalFresh)
  
  All required documentation
  
  Birth Certificate
  
  Baptism Certificate
  
  Immunization record
  
  Physical exam (only for TK and K)
  
  First Communion certificate (if applicable)

  IEP (if applicable)
  
  Please complete and return documents to the school office. We will have new student testing, May 19^th, 2024. We will schedule appointments on that day to sign financial contracts.
  
  Just a few reminders:
  
  There is a mandatory commitment fee of $30 per child is due by September 1, 2024.
  
  Tuition is due the 15^th of each month beginning September 15^th, 2024 via Smart Tuition (all new families MUST sign up for this service). A $50.00 usage fee is required.
